Please bring your medical insurance card to your appointment.
Please bring your driver’s license or another photo ID to your appointment.

PAYMENT INFORMATION

  • We accept cash, checks and American Express, Discover, Mastercard and Visa for all co-payments and payments for services provided.
  • We will bill your insurance company for the services provided.

  • Monday through Thursday – 8 am to 5 pm
